We all know bootleg games have some pretty strange labels... but what ones have you seen that take the cake?
My favorite has to be my copy of T.M.N.T. ... sunglasses and strange weapons FTW! And since when do turtles tunnel underground?? lol

This is the label for "Super Irmãos" ("super brothers in portuguese", a brazilian SMB clone:
A quite interesting interpretation of the Mushroom Kingdom, IMO - specially the badass looking turtle behind "Mario".

Pretty sure I have one of those ^ in the pile. Most of my Brazillian Mario clones say Super Mário "mayrio"?
There are lots of cool labels 'round here, because the brazilian government allowed national companies to produce and sell famiclones and games. Therefore, there are lots of name and label variations for the same game - for example, Gradiente (IIRC) called their Megaman games "Megarockman". The art for those titles is pretty cool, by the way - I'll post pics here if I find any.
EDIT: Megarockman 3, in all its glory:

i've always been a fan of super panda prince myself.
also "super wonderful mario" but i can't find the particular one i'm thinking of. not the one with mario in a star in the centre that you'll probably find on google, i seem to remember this one had a goat on it.
