Better yet, I think I played it.
One of the newer Qix clones isn't mind-destroying in its awfulness, actually, the stuff in that one was rather OK or something (er, Gals Panic S2). The overwhelming sexism in these games is just too overpowering for me...I really couldn't ever be comfortable with them if I tried.
Perestroika Grils...hell on MAME, nearly. With a sexual patina. Leather, chicks, guns, Communism...and no replay value. :o
There was one game I downloaded from KaZaa once...I had no friggen clue how to play it. It was like Qix (except your stuck in a range, instead of making the range), any space shooter, and Cotton.
It was very confusing, and the instructions were in Japanese. It was fun dying though.

X-man on the 2600 is pretty nuts (pun lol). I was playing it on an emulator. It's like a porn star simulator.

I can't believe none of you said Custer's Revenge for the 2600! Sure, it wasn't a very good game, but it was definitely the most famous of the AO-rated games back then. That has to count for something, right?
