Nothing too insane. Donkey Kong Game & Watch and Color A Dinosaur for NES.
A DS game, Emily the Strange, No. 4 of 200 LE copies or Knights of the Temple 2 UK PAL version for the PS2. Have Suikoden, Lunar and other Zelda rare stuff worth more but they're the rarest.
Also rare to me is a game that you can't get, funnily enough because it's rare. Panzar Dragoon, Suikoden 2 and Little Samson are uncommon games (IMHO), not rare. If you wanted one you could go buy one from eBay, it would just be expensive. Hoppie for the Playstation 2 is a rare game for example, mainly because, again, if you wanted one you can't buy one, because it's rare.
Probably the Entex Adventurevision or the two sealed AV games I own.
As a Sega Saturn & Dreamcast collector it'd be my Saturn Prototype console.
Recently I took it out and took several new photos while cleaning it as someone from another forum had been asking me about years. ^ ^
It's not much but the rarest thing I have in my collection is the SHARP Nintendo TV. It's been in the family since it was brand new.

Not completely sure, but my US Sengoku 2 and 3 for Neo Geo AES are pretty rare. I know Sengoku 3 usually doesn't pop up at auction much for a legit US or Euro copy. Usually just conversions.

I don't know if it would be megaman x3 on snes, the vectrix system or the square button famicom manual (1st rev)
As odd as it may be since it's a modern system ever since I picked up the retail boxed version of Retro City Rampage DX that would have to be the rarest game item since there was only a 2000 game print run of that one on PS4.

I don't know how rare it is, considering there appears to be at least 1 copy on eBay at any given moment, but I'll give the nod to Pulseman.
It's a great game, and I encourage anyone with a Wii to pick it up on the Virtual Console. Sure beats the ridiculous amount people are charging for the original MD cart.

Well I got that rare PS4 game a few months ago which had a run of 2000, but I beat that out a couple weeks ago.
Introducing Bally/Midway 1983 - Gold Ball (Pinball machine.) 1750 of them were only made back in 1983 and that's it, it was a rare low run table and also unique as it was the only game to ever have an actual gold (brass) ball to have in play when you meet certain conditions. Initially a random crap shoot of 1-15% of it ever appearing within any games played, but if you have the service kid (or Home ROM v12 which includes that, which is what I have) you can set it to be a regularly earned ball during play. I guess Bally thought better of having a triple score ball almost never show up as it was infuriating to enough people so they made it a skill thing to earn.
I entered the big leagues on this jumping from home pins or plastic tabletop funk but I've wanted one for years, played it at the Louisville Arcade Expo and I had to have it.

Yeah, I don't know exactly how rare it is, but it is definitely getting up there in price. I got a good deal on mine in a lot I got frim a local store that no longer exists. It contained, all complete in box with all of the plastic and paperwork, a Mega Drive 1, Mega CD2, Super 32X, a spare 3 button controller, Sega Tap, and Sega Modem, and about 30 games (all CIB with posters and such) that included some good ones like Pulseman, Sailor Moon, Bare Knuckle II and III, Rent a Hero, Gunstar Heroes, and more, for $200. At the time I wasn't sure if it was a good deal, as I hadn't priced it all out, and didn't know I would be buying it until I got there and stumbled across the huge box it was all sitting in. Turns out I did pretty well!
