This should start you off nicely.

Would also help to know what game types/genres you prefer to play.

Growing up, some of my faves were:
-Toe Jam and Earl
-Sonic series
-Haunting
-General Chaos
-Streets of Rage series
-Golden Axe series
-ThunderForce III
etc.
Shining Force is a classic, don't forget Gunstar Hero, Ristar, Ecco, etc.

I haven't played sega since games were still coming for it, but...
Haunting starring Polterguy- a game where you haunt and control different objects in a house in attempt to scare the family's out. Fun factor was insane for this. someone already mentioned it, but this game is great.
Streets of Rage- again, someone mentioned but I think the SoR series is one of the best beat-em-ups around. 2 is the best.
altered beast- won't be the most obscure game mentioned here but a decent game with a cool concept.
sonic pinball- never ending, NEVER. interesting take on an older concept, worth a couple bucks to check out.
Gunstar Heroes- fun as hell,side-scrolling platform shooting game
alien storm- tons of action, great to just pop in once in a while
ranger x- side-scrolling platformer of sorts. my favorite as a kid, might not have aged well but you play as a mech, cool graphics and fun levels

I never got too much into the Genesis back in the day, I think I only bought something like 15 games for it while it was on the market. My personal favorites were:
Subterrania (kind of like an updated Gravitar)
The Ooze
Outlander (killing bikers with a shotgun is so much fun!)
Todd's Adventures in Slime World (a good port of the Atari Lynx game)
