I'm going to say its from Mega Turrican, as it says so in the image filename.

Well that was my guess but I'm not sure.

It's not a front cover, I'm guessing it's either a back cover or a magazine ad (or just plain unused).

I'm sure I've seen it in magazine ads, but not necessarily ads for Turrican itself, but it is probably from around the release of Turrican 3.
