And by "at once", I mean any console can be used at any time by merely pressing a button or switch. So no plugging in power or AV cables. Right now I have 18 systems hooked up to my TV. I have a bunch more systems that aren't hooked up properly yet because of power or AV/RF issues, but this is what I have so far:
On AV:
Famicom Twin
Nuon
Dreamcast
Playstation 2
FM Towns Marty
NES
SNES
N64
Gamecube
Jaguar
SMS
Genesis/Sega CD/32x
Saturn
Panasonic FS-A1 (A MSX 2 computer)
On RF:
SG-1000 Mark II
Colecovision
Intellivision
Odyssey 2

Nothing hooked up right now.
I'm cleaning my room and moving things around to be Feng Shui. :D
What I'll have setup on my TV are:
Atari 5200
Sega Saturn
Nintendo GameCube
PSX
Coleco plug-n-play
Yup, 4 systems will be connected at a time. I do have a PS2 hooked up downstairs for DVDs and PSX/2 games. Now there are a few other things hooked up too:
Cable box
DVD/CD/VHS player
LD/CD/Karaoke player
Record player (just uses an audio port and transfers to my receiver)

I wish I had an entertainment center large enough for me to plug in all my consoles at once, and then just switch off that power strip to conserve electricity until I want to play it. Right now, I've got the 360, Nuon, GameCube, Xavix, and Dreamcast set up. I've got some other machines close by in case I feel like playing others, and I have a few in storage, waiting for their chance.

Right now I have:
1. Playstation 2
2. Xbox
3. Dreamcast
4. Nintendo Entertainment System
5. Super Nintendo Entertainment System
6. Sega Genesis
7. Sega Saturn
8. Sega Master System
9. Nintendo Gamecube
10. Turbographx 16
So I'm up to ten consoles, other than the PS2, XB, and GC they are all hooked up threw RF connectors so I have a long line of RF cables. But I dont have all the power supplies pluged in at once.

-Super Famicom + SNES Convertor
-Sega CDX
-Japanese Saturn (All Region Modded)
-Japanese DC (All Region Modded)

I am waiting to have a permanent residence to do all the AV Modding as well as running a setup with HDTV for the stuff that is best with it, and an RGB Monitor for stuff that looks best for that. More than an insane software collection, a killer hardware setup is my main goal at the moment.

A few Years ago,I had a ton of systems hooked up..it became my main goal to have as many of them hooked as I could,..switches and switches, power strip boards, long ones that go across the walls..When something became problematic,It became such a mess of wires that I just had to undo it all...I got too frustrated with it and felt it might be a fire hazard.(someone stuck that in my head)
Some pics of the set up might be in my collection page.
It took me a day just to unhook it all..this wasn't a fun chore to do either and I promised myself that I'll never do that again.
Just the power supplies alone, filled up plastic bin ..what a pain in the ass to wind up,deal with..
Right now, what I have hooked up to two TVs side by side(my Son plays his own thus I own two of things)
PS3
Wii
Two 360s,though one is being repaired.(red lights of death)
Three Xboxes(I love link games HATE split screen)
Two PS2s
Two N64s,one modded for Japanese games
Two Sega CDs,(different versions..One with 32X on top-both units going to be for sale..
Genesis 3(because it so small)
Gamecube
Playstation(my launch system)PSone ontop of it,but I unhooked it..
NES toploader(lol I sell toasters but don't have one hooked up
Famicom
SNES mini system
V Smile
The above wasn't all hooked up with a goal to do it..I just kept finding spots to plug them in and it happened.
Honestly, with my older stuff I don't mind plugging.hooking it up to play it if I had too..

The set with 18 is plugged into a wall outlet which is controlled by a light switch so I can turn the power off -- there is no power drawn when it's off that way.

I'm probably last with only 3 :( But hey, at least they're all on separate connections :D
Saturn - S-Video (No RGB Scart in Japan anymore :()
Dreamcast - VGA (Via the VGA Box of course)
Wii - Component
All my other stuff has to live in the dark underground dungeon only to see the light of day once every two weeks if lucky.

Right now I need to get another AV switch and another power squid so I can add a few more systems. I'd like to get an AV switch that supports more than 4 systems, but they're usually too expensive.
Where I'm at an impasse is with RF. I've got one of those 4-to-1 RF adapters so I don't have to daisy chain everything, but the more systems you hook up to it the worse the picture looks on every one. I'll probably buy an A/B RF switch and put a 4-to-1 adapter on each one.
My last problem is with Japanese RF based systems like the Cassette Vision, Super Cassette Vision, Gakken Compact Boy, etc. They all come in on weird channels on a US television, but my current 25 year old TV doesn't go past channel 60 where most of them show up. Not sure how I'm going to resolve that one short of buying a new TV. Thankfully my SG-1000 comes in on channel 32, and I understand it's no big deal to AV mod the thing. But I intend to retire the SG-1000 when I either mod my SMS to play SG-1000 games or when I buy a proper Mark III or Japanese SMS.

Well, if forced to comply strictly by the OP's rules then I have none because I never let any of my systems to be plugged into a power outlet unless I'm using it at that time. So, naturally that precludes having them all hooked into a power strip. I do have several connected to the main TV, via RF:
Atari 5200
NES
Sega Genesis/CD/32X
Atari Jaguar
Since only one one of the VCR's have front located A/V input I can only have one system hooked up that way at a time. Currently the Sega Saturn is in that position. I've been working my way through Norse By Norsewest for some time now.
As for other systems with dedicated monitors/TV's there's quite a few:
Mattel Aquarius
Commodore Vic-20 and a Commodore 128D both connected to a single 1702
Atari 1040STf
C16 and a Plus/4 hooked to an old Magnavox amber monitor until I get another 1702 for them.
Apple ][C
Apple ][E and a C64 to a single 1702
TI99/4A
Tandy CoCo3
Atari 800
Plus three old PC's including an old 386 system doing various gaming duties.

The question I keep asking myself is, do I really need to have, say, a Casio Loopy or Apple Pippin or Amstrad GX4000 wired up so I can use them any time I want? How often does that happen? Even if I have the space, I could probably use that space for something better. And it involves more wiring, more switch boxes, and making everything more complicated.
It sure is nice thinking "I want to play Turpin" and being able to just turn on my Odyssey 2 and play it without having to dig through a pile of wires to plug it in, though. Once I have everything wired up to my satisfaction, I'll probably just leave one AV port, one RF port, and one power socket available so I can plug in my rarely used systems easily when I feel the need to.

I used to have about 12 consoles hooked up at once but my problem was I'm always bringing something over to a friends house or whatnot and it used to be a hassle to disconnect and reconnect a console. I tried bundling cables in various ways but they still always managed to get tangled up. What I have ended up doing is just having all my consoles out and I use ziplocks to store all the cables and memory cards for each system. Its all labeled so when I want to play something I find the ziplocks that pertain to it hook it all up and go. If a friend wants me to being over my Genesis or my N64 or something I do the same, just grab the stuff and go.
