Actually in an operating arcade? I've seen them in converted home machines, but not in a working arcade environment.

Flashbacks in Seaside Heights, NJ has one, although I don't think there is anywhere near 400 games. Maybe 100? Frankly for a Multi-cade, I never heard of most of the games on it.
