Are there any games that really stood apart from the 5200? Are there any original titles exclusive to this system worth getting?

I don't know how Joust compares to the 5200 but I thoroughly enjoyed it on the 7800. Some have said it is the best port of it before emulation came about.
Xevious is the shizzle. Yes there are better looking ports out there with better sound (Xevious on PC-Engine comes to mind) but the 7800 port is super excellent given the hardware it's running on.
I also prefer Robotron: 2084 on the 7800. While it lacks the Pokey sound chip of the 5200, I much prefer playing without the 5200 joysticks.
Commando is also said to be quite good but I haven't found an affordable copy yet.

Food Fight seems to be the common answer.... it's pretty great.

I'll second Ninja Golf. Midnight Mutants was a unique game as well (adventure rpg with reasonable graphics on an Atari? ya rly). Tower Toppler is kind of fun. And I'll highly recommend Dark Chambers (which was what inspired the creation of Gauntlet although most people think it was the other way around).

I spent most of the time with the 7800 on Dark Chambers, Dig Dug and Ninja Golf. There's nothing like Ninja Golf anywhere else, and the whole concept is just hilarious. I'll second Xevious also.
I played Choplifter on it quite a bit as well, but like the Colecovision version more (I don't have a Coleco anymore though).
