Can't go wrong with Gunstar Heroes on the Genesis for co-op. Mortal Kombat for head-to-head.

TMNT IV is my personal favorite for co-op on the SNES, Faceball 2000 is good for head-to-head, along with Street Fighter II.

Super Mario World's kinda fun when 2 play... Also I like Ms. Pac-Man and Mr. Do! (both have 2 players play at the same time.)

One of my personal favorite two-player games of all time is ToeJam & Earl (Genesis)... can't go wrong there.

Street Fighter for SNES...I don't think there was a bad version. Mario Kart SNES, not sure about sega stuff.

Zombies Ate My Neighbors (for either system).

Columns III is the best two-player puzzle game. The "attacks" are more rewarding.

Im partial for ZOmbies Ate My Neighbors and Super Smash TV for the SNES. I really liked the Punisher for genny.

GoldenAxe 1 and 2 for the genny are good, and of course Contra 3 for the SNES is awsome.

Streets of Rage 2 or Punisher for the Genesis. You also have Golden Axe, Battletoads Double Dragon is not to shabby either. Genesis does brawlers

if you want a good 2 player challenge then you can't go wrong with contra for snes and genesis

My favorite games:

Genesis: Streets of Rage Trilogy, Golden Axe 1 and 2, Street Fighter II Special Champion Edition, Two Crude Dudes, Contra Hard Corps and TNMT: The Hyperstone Heist

Super NES: Final Fight 2 and 3, TNMT: Turtles in Time, Contra III: The Alien Wars and Street Fighter II Turbo

If you like sports games, Capcom's Soccer Shootout on the SNES is a must, even if you hate real soccer. I've played this more in two-player mode than probably all other games combined.

Toejam & Earl on the Genesis is my favorite two player game of all time. I also like Zombies Ate My Neighbors (GEN/SNES) and Herzog Zwei (GEN) a lot.

Of course, there are a lot of other, random ones that I like to pop in from time to time. Both Looney Tunes B-Ball and Tiny Toons Wacky Sports Challenge (both SNES) can be fun. There are tons of more overlooked ones like these.

Genesis:

1. General Chaos (legendary game that needs to be made on current gen systems)

2. Columns

3. Golden Axe

4. Bonanza Bros.

5. NHL Hockey (some years better than others but all still fun on multi-player)

SNES:

1. Uniracers

2. Street Fighter 2 (pick your version)

3. Killer Instinct (kind of bland on SNES but still fun multi-player)

4. Mario Kart (The KING of multi-player!)

5. Bomberman 1 to 758 (The KING of multi-player if it were possible to have two KINGS at once)

I'm going to throw a vote for the underrated Eternal Champions on the Genny but otherwise most of the greats have been mentioned on here.

Some other "Sports but with a twist" games I like:

Mutant League Football / Hockey (GEN) - why have we not gotten a sequel?

Mega Man Soccer (SNES) - Just a total favorite

and of course, the import Super Dodge Ball for snes... good times. :D

Since most of the other games were cited.

Cadash for SEGA Genesis.
YuYu Hakusho Sunset Fighters (TecToy release of the Treasure game), this game was localized for South America. But since the Brasil is part of the America Continent the game should play just fine. For example, in the Majesco cardboard boxes of games it is written that the game will run on Brasil. Even if most South American releases do not have anything written most of them are compatible with North American cartridges, the SEGA CD and SEGA Saturn games were the same SEGA of America discs ...

Gangster Town if someone use a SEGA Power Base Converter and two SEGA Light Phaser guns to play this game.

NBA JAM T.E. is a nice multi player game.
Street Racer is also nice to play. But this game is not a cooperative game.

Even with just two players Micro Machines 2 on the Genesis (okay, Mega Drive, as it was only released in the UK) is utterly compulsive. Especially as you can actually plug extra joypads into the cart itself to turn it into a 3-4 player game. :)

Sunset Riders on SNES. The Genesis version isn't as good, but still fun to play. I also would like to second any of the Micro Machine titles. Very fun multiplayer games. Zombies Ate My Neighbors is also a great choice.

Yeah, most people have stated the obvious titles for both the SNES and the Genny. Here are a couple others:

SNES
Tetris/Dr. Mario - this is by far the best version of 2-player Tetris on any console (until Tetris Party on Wii, anyway). The classic Gameboy music is suped up and the graphics are bright and vivid!

Power Instinct - Everyone gave a shout out to Street Fighter 2 and Mortal Kombat, but this quirky tournament fighter HAS to be mentioned! Ever play a game where you fight a grandma that shoots her dentures at you? 'Nuff said!

Saturday Night Slam Masters - this is on the Genny too, but I prefer the SNES version. Technically a FOUR-player game, but great as a 2-player wrestling game.

I didn't play alot of 2 player games for the genesis but there was a couple.

Punisher was very fun with 2 players.

X-Men was also another game I liked with 2 players, Not sure if X-Men 2 Clones Wars is 2 players (Never played it sorry) but if it is I assume its just as good if not better then the first (I heard its better so take it for what its worth)

Yes, some of the early arcade ports to home consoles were great to play !!! Even better their still have the two players cooperative mode ...

Alien Storm is another game from SEGA that resemble the old Golden Axe formula .This time with shooting sections! To bad that those games are only on the SEGA Genesis.

One game that is on the SNES is The King of Dragons. This game is nice for two players.

Those 16-bit machines, much like other home microcomputers of that time were very nice for arcade ports ...

Goof Troop for snes is pretty fun. It plays like a zelda game without the epic or swordfighting.

Battletoads Double Dragon - for the 'kick your buddy's ass because he missed the bad guy and his flying kick got you instead'
NBA Jam - 'hes on fire!"

SNES:
Donkey Kong Country - because it Donkey Kong
Gradius III - a classic shooter that always delivers
Ranma 1/2 Hard Battle - for the sound effects :)
