On TV Tropes's "Continuing is Painful" page(http://tvtropes.org/pmwiki/pmwiki.php/Main/ContinuingIsPainful) I found this entry:
# Thought Sierra's games were cruel? In the 1980s, Sierra released a flight simulator in which if you crashed your plane, it deleted your entire game. Not the save. The game itself. Wiped from the floppy. The creator (Ken Williams) said that such would be "close to those same death-defying stunts". This triggered such an angry outburst from the public that the game was removed from shelves and Sierra never made mention of it again except for a footnote or two.

Anyone know what game that is, or any more information about it?

I've always wondered what a game like that would be like, but I never thought one actually existed. I'd love to find out more about this.

I don't know the answer, but this list is sortof flawed.

In Breath of Fire Dragon Quarter, dying only forced you to completely restart at the beginning of the game if you didn't save previously. If you saved then you could restart with whatever party exp you had at the save point(party exp being distributable exp. Characters do not keep regular exp gained.)

With Mega Man games, you don't lose any energy for your weapons when dying. However, you don't regain any used throughout the level once dying.

I find this hard to believe. Half the fun of the early flight sims was to find cool things to crash your plane into.
