Star Fox, Outrun, etc.

If you exclude titles that are quickly finished like those and actually require more than a few minutes of time, Super Mario World is easily the winner for me.

Definately Star Wars: Battlefront.

I will play that from start to finish from time to time. The game just never gets old for me.
Also, Shenmue. I have played through and beaten countless times

I've beat Resident Evil 4 somewhere close to 25 times in the past 4+ years. 17 of those were in 2005 (mostly speed runs on the GC). Gamecube, PS2, PC, and Wii versions...I have them all. I'll admit, it's kind of strange.

In The Groove. Been playing for an hour almost every night for the past 3 1/2 years. I mix it up by playing the JP and US DDR games too but ITG is the one I play the most.

There have been quite a few i've beaten 10+ times.

Twisted Metal 2 has to take the cake though.

Wanting to see every characters ending and then playing co-op with friends (which never gets old) in total I have probably beaten the game 50 times.

Remembering the D-pad combos is like riding a bike, you never forget! Even if I think I have forgotten how to pull off the combos my fingers always remember ever single one of them with laying land mines being the least remembered, lol.

Although I haven't played it in a few years, it would probably be Goldeneye. Not only did I love the multiplayer, but I've played through the single player so many times that it's hard to count. Maybe 3 or 4 dozen?

Tetris on the NES. I get on kicks where I'll play every day for months (about 30 minutes a day)

If I had to guess, discounting beat-em-ups, probably Sonic 3. If you count it when it's attached to S&K my completion count is well over 50 and possibly pushing 100. I remember my old cart where I'd managed to fill each save slot with different completion criteria (Sonic, Super Sonic, Hyper Sonic etc).

If you count run-throughs of beat-em-up arcade modes, probably Soul Calibur 1 or Tekken 3. Well over 100 for each, probably pushing 200 for SC1.

For me it would be Sonic 2, Mystical Fighter, Golden Axe and Goldeneye.
I can play those games a million times and they will never get old.

Other than that - Zelda II. But I'm rather obsessive about my Zelda II.

Super Mario 2 more than any other for some reason, come to think of it.

Mega Man 2 and DuckTales - got them the same christmas, alternated through both.

Chrono Trigger I've played through for all the endings, but that might not technically count as "from the beginning" each time with New Game +

Pilotwings for SNES... ran through that whole game at least 20 times back when I got it.

Unlike most folks who posted here, I can count the games I've played through multiple times on one hand, most likely. Off the top of my head:

Banjo-Kazooie (3)
Metal Gear Solid 2 (2-3)
Deus Ex (2)
Donkey Kong Country 2 (2)
Fallout 3 (2)
Final Fantasy X (2)

That's all I can think of, but there's probably a couple more. I'm not including crap like Golden Axe which can be beaten in 12 minutes.

My most played games are Aoe3, Halo 2 (easily 700+ hours each), Halo 3, Gears of War 2, Age of Empires 2 and Age of Mythology. All online.

Deus Ex [PC] and Final fantasy 3 for me. I played through FF3 atleast 15 times since it came out, mostly a few years ago...still the best Jrpg ever released imo. Only downside to it really is the game balance goes to shit in world of ruin, since everyone can learn every magic there is with enough patience/time. Despite that though, the story is good, even by todays standards for the most part. And the game play is great.

Deus ex, another great game! easily one of the best, and most underrated games there is...even though most sites gave it all 9's, little was said about it compared to how much press the half lifes/quakes got during its release. I played through that about 8 times or so...but like FF3, game is awesome most of the way through then kinda falters/feels rushed for release towards the end.

Both are worth playing even today, specially Deus ex if you like RPG'ish type gameplay mixed into a FPS..works really well in that game believe it or not.

I know I've played through Super Mario Bros. on NES and Sonic the Hedgehog on Sega Genesis over 100 times if not more. They were my first two cosole games and I'm still addicted. I will probably complete them both over another 100 times in the future.

For the amount of times played, it'd be Castlevania with 200+ atleast(short game) and Super Castlevania 4(same amount, around that.)

For length it would be the original Final Fantasy. Sure I've played games that are extremely lengthy like Tactics Ogre atleast 10+ times(50-100 hours,) Xenogears atleast 5+ times(30-50 hours,) etc, but with every version of FF1 combined the amount of times I've played through the game is around 100 times(it's 5-10 hours depending on the version, with the Dawn of Souls version and Anniversary Edition topping out at 10 hours because of the extra dungeons.)

I love the original Final Fantasy. Not even my favorite game but I always go back to it.

It's still Super Mario Bros. I racked up enough play on that as a kid that I don't think I'll ever top it, I must have beaten it dozens of times. I beat it about five times in a row once (okay using warps), just to see if there were any new varieties beyond the original hard mode with the beetles.

ESPN NFL 2k5. Up until Madden 09, I just updated the rosters every year. I still prefer it over Madden. I'd still be playing it if my X-Box controller didn't break.

I also put many hours into Diablo II: LoD, FFVIII, and Virtua Fighter 4.

Super Mario Bros and Legend of Zelda are probably #2 and #3. Final Fantasy is a contender as well. I am actually currently playing through it again via Origins.

For me it's gotta be SMB3....had a big long revisit to that one a couple summers ago, where we had an NES set up on an ex-gf's porch and a bunch of us used to sit out there together 3-4 nights a week getting wasted and playing that game through, often leaving it on overnight to finish the next day, I think there was some kinda unspoken "no warping" law....

It got to the point where it wasnt a question of "can this trio/quartet of drunks beat the game?", but "how many lives will we have left at the end?"

Easily PSO Ep 1&2. Followed by Gran Turismo 1, 3 and 2...in that order. SO much time (years) on PSO. Turismo was the greatest game for a race nut...nothing else comes close to this day.

I actually rarely go back to finish a shmup again after I've played it through. I'll play the heck out of them, but it's more a thing to pass the time than to finish it. For example, I played Gradius Galaxies for several weeks after work, waiting for my dad to leave (didn't have my car back then). Once I finished it, I still played an hour a day it for several more weeks, but I haven't played it from start to finish since. In an unrelated note, that's probably my favorite "replay value" game, largely due to the good save and continue feature.

Fighting games, I'm not sure how you'd classify them. Would you have to play through the entire ladder, or just one fight? If it's just one fight, then by similar rules, Crazy Taxi is my #1 most replayed game ever. I have a much longer atttention span for Crazy Taxi than any fighter.
