This is going to be interesting guessing what well see up there and which one takes the grand prize.
My guesses will be:
John Romeros over hyped Daikatana
and
Nintendo backing up from the CD addon for SNES and so helping in the creation of PSX.

Well, the Nintendo part is usually seen as an obvious choice, but I'd like to say that this assumes Nintendo would get the sort of business the Sony console got. I wouldn't compare the N64's third party support to that of the GameCube simply because today we have two other very powerful manufacturers to contend with.
At the time it didn't seem THAT bad a tradeoff, since good, dependable CD-ROMs just weren't easy to come by. Nintendo likes to make a console that's darn near indestructible, and it made more sense back then since relatively few people used data CD-ROMs with any regularity (I myself managed to scratch up the Quake CD's MUSIC tracks to where it was giving out different sound...how far we've come since).
It was simply a concession --quality over quantity and lower price --that I myself think could have been a winner if the system had more good games.
The other obvious choice for a Dumb Moment would be Atari finding some small fault with a Nintendo character that supposedly was a ripoff of one of theirs, and declining to market Nintendo's NES console. Oops.
Another obvious choice would be...hmm, the time when Commodore decided to NOT release a color computer. Well, Commodore was screwed anyways...but that would have been a good choice.

And what about Duke Nukem Forever? That I guess isn't quite dumb yet, as there's still a small fraction of a chance that it'll actually see store shelves.

Gah, I enter the GameSpy Forums and are greeted with some post that proclaims the Turbo Grafx 16 was NOT 16 BITS!!!!1111
I used to have fun there, but whenever there's a topic like this all the idiots come out and proclaim that such-and-such obscure game's poor physics is one of the BIGGEST VIDEOGAMING MISTAKES EVER! Somebody's complained about Test Drive's physics, for goodness sake...
Anyways, I'm enjoying the articles. I don't like seeing the E.T. one, but it's there.
