Focusing mainly on Genny and SNES here, how many of these are around. I can think of a few but I'm wondering if there's others. I'm not looking for ports here, just games that have the same name but are actually two completely different games..

Indiana Jones and the Last Crusade (not just on NES)

Battletoads is the same game, just with slight differences (the Genesis version is easier than the NES version).

I'm not sure how to link to other threads but we had a pretty comprehensive list going a couple of months ago. The thread was similarly titled "Different games with the same name (Genesis and SNES)".

There are quite a few on the list. Hopefully that helps.

How different do the games have to be? Would Tetris count?

And then there are games like Castlevania on the N64. It's simply called "Castlevania" but it's obviously very different from the NES game.

Theres a Crackdown for Genesis/Megadrive and then theres the 360 game.

Don't forget about all of the Mega Man games on NES and Game Boy. There was a Mega Man 2, 3, 4, and 5 on both systems.

The handheld games use roman numerals though.

Good point, but the NES games use Roman numerals on the title screens, so I'm not sure it's that big of a deal.

The Addams Family for NES and SNES are different games, but Pugsley's Scavenger Hunt for the NES is a port of the first SNES Addams Family game, while Pugsley's Scavenger Hunt for SNES is an entirely different game. Confusing stuff.
