Continually...
Press fire for ball&chain
Push left to avoid jug
Press fire to push the wall back
Push right to dodge the axe, right again to dodge shield
Press fire to push back the other wall
Push left to dodge another axe
Press fire to dispose of another shield
Push forward to avoid another axe
Push right to dodge fireball before you exit
Hope that's right ;)
