How can you tell a fake Japanese Mega Drive game from a real one?
and how can i play it in a US Genesis?

Best way to tell a fake is to open it up and look at the chips (or lack of) inside. Usually the pirate carts have a glob instead of a chip, but some pirates do have chips too. Also, many times the Label will have a difference or not look as nice and glossy. Sometimes without being there in person to look at it, it might be hard to tell. Just make sure you can get a refund or have them pay to return it if you verify it is a pirate.
As far as getting them to work in a Genesis.
A good portion of them will work without a region switch. Some won't work because of region lockout - with those you can either install a region switch, or use a Game Genie.
The carts won't fit in a Genesis unless the cart slot is widened a bit which is pretty easy to do, it's similar to modding a SNES to play SFC games.
The X'Eye console a cart will fit in without any modification and will play games, but if it's got region lockout you'll still need a region switch or a Game Genie.
You don't have to widen your cart slot if you have a game genie, the game genie acts as sort of a pass through and oftentimes you don't even have to put in a code to make it work, you just hit start.
There is a master list of all the game genie codes to make all region locked games work.
Game Genies are cheap and easy to find on Ebay.

I've been wanting to add that to my collection for some time. The repro for $45 looks tempting and it's NTSC-U

A lot of people claim to have been scammed by that guy, as far as I've read on different forums. Be cautious.

I've bought a few repros from electrochip, and a region/svideo modded X'eye from him. No issues.

I didn't scam anyone, I had some issues with hardware and was backlogged because of it. Everyone has received there games, I think 2 are still on there way to CANADA.

As I said, I'm just reporting what others have said. If you say it's all cleared up, then I'd have no worries with you. Always good to be cautious, of course.

If you are using Paypal, you are protected. If the seller does not come through, you can file a claim and get your money back.
