I'm trying to compile a list (at least of US releases, I'm not worried about overseas just yet), and I've only got a few. Keep in mind, these don't include later system releases, just the first runs (so the Sonic pack-in with the Genesis doesn't count, for example).
Here's what I have so far. If you know some more, post them here, and feel free to correct me if I'm wrong on any of these - or even point me to a place that has all of these listed. In the case of the NES and SMS, I recall that there were several different sets released at the same time that had variations on the pack-in. I've listed the two that I know of off the top of my head.
Combat (VCS)
Las Vegas Poker & Blackjack (Intellivision)
Speedway/Spinout/Crypto-Logic (Odyssey 2)
Pole Position II (7800)
SMB/Duck Hunt (NES)
Hang On/Safari Hunt (SMS)
Tetris (Gameboy)
Keith Courage In Alpha Zones (TG16)
Altered Beast (Genesis)
Super Mario World (SNES)
...and that's all I can think of so far.
