Man, wish I could help out but I got nothing on your list. Now that I think about it, I bet that RE flat cardboard version is quite rare (if it exists?), because I've seen at least 100 hinged versions without a cardboard one.

I saw that card board long box RE at a thrift store about 6 months ago. Had no disc inside so I didn't get it. It does exist.
