(in no particular order)
Battle of Olympus
Kabuki Quantum Fighter
The Flintstones: The Rescue of Dino & Hoppy
Barker Bill's Trick Shooting
Tiny Toon Adventures

EDIT: A little clarity-- I don't mean underrated gems or anything like that. I just mean games that you like for whatever reason, and nobody else seems to. Not necessarily good or even average games. More of a "these are my guilty pleasures" kind of a list.

Kickmaster is an often overlooked gem on the nes. It is really a neat little game that was released late in the life of NES that had outstanding graphics and good gameplay.

I really like Mighty Bomb Jack. Seanbaby has it on his list of the 20 worst games, and really that's about the only place I've seen too much of it online.

Dr. Mario doesn't get enough love. I love that thing, although I have to admit that I play the SNES one with Tetris more often.

Solar Jetman= I don't love it (yet). I've played it only until third stage or so, but I already know that is a very complex, original and atmospheric game.

Tecmo Cup Football Game= The occidental version of Captain Tsubasa. I enjoy its strange mix of sport+rpg a lot. In fact, my nick comes from this game.

Jack Nicklaus 18th bla bla bla...= Only one course, and the graphics take forever to be generated on-screen, but I find it a lot of fun playing it from time to time.

Elite= I wish I'd know how to play it. It's a classic on computers and you don't get to see games this complex on the good ol' Nes.

Flintstones= A very competent platformer.

If I had to pick five games that I love that no one knows or cares about I'd have to pick

Treasure Master - American Softworks
A very hard but underrated platformer. Love the music and graphics.

Flying Warriors
Great Sequel to Flying Dragon with a very unique fighting system.

Attack of the Killer Tomatoes
Love the atmosphere in this game and despite what others say it has a lot of variety in the stages and I never had trouble mastering the controls or controlling the game properly.

Labyrinth (Famicom)
Game based off of the 1986 David Bowie and Jennifer Connelly movie that incorporates the aspects of the movie into the game marvelously.

Code Name Viper
Great clone of Rolling Thunder set in South America that never gets noticed whatsoever.

You know, I loved Bucky O'Hare and I got it when I was a kid because I loved the cartoon. Never got close to beating it though. Such a challenging game. I should try it again one of these days.

No surpise there given what little samson go's for these days you won't see many,talk about it.Heck i didn't even know about the game untile a few years ago.From what i understand it was a late release in the nes lifespan befor...the console died out.

my 5 game list would comprise of

5)Hudsons Adventure Island (people do talk a little about it but a great game)
4)Kung-Fu (its five levels of 20 min fun, great game)
3)GunSmoke (why this game isn't talked about is beyond me)
2)Blaster Master (one of the most under-rated games ever)
1)Solar Jetman (there should be a sequel and the fact that there isnt tears a small hole in my soul)

So thats my top 4.5 that no-one talks about, I just feel Adventure Island for its time is extremely underrated.

No one talks about Solar Jetman because no one can finish the darn thing. It's such a great game, but it's not at all accessible by today's standards. After the first few planets, it's so bewildering that you really don't want to bother with it anymore. Still, it's such a great concept, and I would love to see a more balanced sequel.

In no particular order...

Guerilla War - I guess I've seen one or two other people mention this game in this topic, but outside of that it doesn't get much love. Cool Story Bro: I found this game at a garage sale when I was 14 or so. And my best friend at the time came over and we played through it in one try (since it has unlimited continues) while listening to the entirety of Rush's Grace Under Pressure album, which was also the first time I ever listened to that album.

Alpha Mission - I can't remember when I first played this game. I remember being surprised that it was so fun and I had never heard of it.

Shatterhand - I loved this game enough to name my band in high school Shatterhand.

Crystalis - I guess this game get's talked about some, but I never played it or even heard of it when I was younger.

Destiny of an Emperor - Once again, another awesome RPG that I didn't hear about until years later when my friend got me into Dynasty Warriors 2 on the PS2.

These are just the first five games that popped into my head, I'm sure there are others that are equally as awesome.

Before I forget: some folks mentioned Rockin' Kats, Megaman 4, DuckTales, Bucky O'Hare and Adventure Island II. I totally agree with them. Awesome games!

Totally Rad: a pleasant, easy platformer with a memorable tag-song.
Bayou Billy: a beast of a challenge, but a very cool potpourri game ( sidescroller, gun-shooter, driver a la Hang-On/Grand Prix). I believe the Konami code has some effect.
Renegade: not great, but endearingly unsuited to the NES.
Sky Shark: the best of a horde of Taito offspring and clones--half an era played like this. Examples of kin are Fireshark (Genesis), Daisenpuu, Arrow Flash (Genesis), and Dragon Spirit.
Lee Trevino's Fighting Golf: another black eye on the Nintendo advertising legacy...there were so many of these that delivered absolutely nothing of their promise. There's no fighting, and Trevino's not in it. What you get is echt Nintendo: plenty of orange and green, confusion and frustration. But it has great oddity cum period appeal.
Xexyz...I really don't ken whether this's well known or regarded, but it's one of the very best games on the system, perhaps second after the ineluctable Mario 3.

Solar Jetman was going to be on my list, but seeing how many others have it listed I'll replace it with another. DuckTales and Chip n Dale are always mentioned, but are two of the best NES games so deserving of praise.

-the Magic of Scheherazade Zelda style action/adventure and turn based RPG in one.

Seriously, I used to play the living hell out of that game, and I still think it holds up decently. I think a lot of the games don't hold up well at all now, but that one does, and yet no one talks about it and you can find it everywhere for like two bucks.

- Magic of Sheherazade
- Crash 'n the Boys: Street Challenge
- Startropics

Some of the system's finest hidden gems.
River City Ransom is sick too, but pretty much known throughout.
I guess I'm a sucker for kunio-kun games.
