Basically whats in the thread title. :)
I'm wondering if there are any great Japanese Sega CD games that we didn't get here or that we did get but are better in that region.

It depends on what kind of games you are looking for. I have a good amount of Japanese Mega CD games and the ones I'd recommend are Final Fight CD (uncensored: shorter skirts on the girls and shirt flies up when you hit them) and Urusei Yatsura. Although Urusei is text heavy, you can still make it through it with a basic guide. The game also showcases, along with Lunar EB, the best animation the system has seen. Even more text heavy, though, is 3x3 Eyes which is also a good game..

Yeah. As said, if you can read/understand vocal Japanese and are a fan of the anime, pick up Urusei Yatsura: My Dear Friends. Game Arts at their best.

Ummmm... Earnest Evans is a JPN Mega CD exclusive, although it did come out on cart (without animated scenes) in other regions. Ninja Warriors was already mentioned.
Aside from the text-heavy games, there really isn't much.
Sengoku Denshou is good, if you can find it for an ok price - good side scrolling beat em up. Bit easy though. There's also Anet Futatabi, which is much harder but a tad crappier - stars the girl from El Viento (and Earnest Evans, a game I never cared for much).
Devastator's a Jp exclusive shmup - I wouldn't call it a must-have though.
I also quite like the Game no Kanzume collections, which contain some unusual modem-only games (and Flicky again). I've not seen anyone else who gives a crap about these, so perhaps I'm in the minority - some of the games are definitely worth a look though, imo.
Oh, and Sonic CD - the Jp (and Euro) game has a better soundtrack than the US one.

I'll second Night Striker. Fantastic arcade game, even better on Saturn, decent on Sega CD.

I actually own it and I find just ok. Kinda was hoping for more as the last Mega CD game made as well as the pretty good storyline and setting for the game...

I remember when I finally bought the Japanese Sonic CD and was able to compare the music and, you know what? I didn't like it! Well, I should say I didn't find it any better than the US one and was nowhere near as infintely better than the US one. Some tracks I like more in the Mega CD version and others ones I like more on the Sega CD version. The opening track for the Mega CD version though? Dear God that is awful! lol

I wasn't particularly impressed with the OST in the JP version of Sonic CD, but I found the OST in the NA version to be insufferable.

I looked around online at the games suggested here.
The animation they got out of the system with many of the RPG/Adventure games suggested was impressive, however I only know a few words in Japanese. Its a shame because 3x3 eyes, Urusei Yatsura, and Shadowrun really look interesting.
I'll probably end up picking up Sonic CD, Final Fight CD, and Night Striker in the near future.

How can you not like the opening theme, "Toot Toot Sonic Warrior"? I mean, it's so perverted that it's funny! Just look at the lyrics. At :29 of Toot Toot Sonic Warrior, it REALLY sounds like they are singing:

Fat tits!"

It's not so bad, if you can laugh at it. And it's cheesy as anything but I still think it suits the intro better than the tired sounding "Sonic Boom". Plus its based on Sms Sonic 2 Green Hill music, and if you don't like that then it's possible you have no soul ;)

I didn't even know the US ost was different until I heard it on the Sonic Gems collection. Some of it's ok (boss music / game over = terrifying :d ) but most is forgettable to awful (that end of level music, bleh). Plus it was a lazy job, since the "past" music is still all from the Jp ost. (yeah, I know why, but still irritates me.) Whatever, it seems if the US version is the first one you heard then you'll probably always prefer it, and vice-versa.

Oh, I never knew it was based on the SMS version of Sonic 2. I just had a listen to it and, yes, it does sound nice on the SMS :) I like it as the title screen music but I was referring more to the horrible animation "rap" song. If if I look at it as just being cheesy, it doesn't help! It's just horrible. I would easily take the "also-cheesy-but-in-a-different-way" Sonic Boom. In any regards, I just feel each soundtrack has its' own strengths and weaknesses..
