Was this in fact only distributed through Volvo dealerships? I saw an "initial" production figure of 100,000 in an old NY Times article, though the article does refer to the game as a "cartridge". Is this a case of a promotional game that may be harder to find but that no one really cares about?

It isn't going for a whole lot on Amazon. I have a sealed one that I have been holding on to in case the value ever spikes.

I doubt we will ever see much of a price spike on this one. It was hard to find around launch, mostly because many Volvo dealers had no idea what it was and it required dealers to proactively order some to stock them in their parts department. Many just never bothered to order. Volvo gave some away at auto shows, but around a year after release, they just dumped them all on a discount distributor and all the independent game shops in the Los Angeles area got them in. I also saw tons of them pop up on Ebay. I still constantly see them factory sealed at various local game stores to this day. I also see plenty of used copies at swap meets and used game shops that carry Xbox stuff.
