A simpler answer to a semi complex question... there never was.
My guess it was programmers looking to make it an old game experience. You unlock it when it is appropriate.

On this same note has anyone used the Move on the PS3 version of Gold Edition? It looks like it could be really fun with a friend and a couple of light gun-like attachments.

All DLC is unlocked from the start. There's nothing to download. It's all on disc.

Again. All DLC is unlocked from the start.
Everything that has a question mark beside it is not DLC. All of that has to be unlocked on both the regular version and gold version. For example. Max out every stat with each gun, each gun will have a purchasable infinite ammo instead of question marks. Shoot the BSAA emblems in each stage and they unlock more question spots. Usually extra outfits, etc.
All purchased outfits, the extra mercenaries mode, versus, Lost in Nightmares, and the other one are the only things that are different than from purchasing the standard version. All of those are unlocked at the very start.
