Ascii Fighter Stick SG-6... was a damn fine controller until I smashed it straight down on the floor in a fit of Ecco 1-induced rage, and the joystick broke off and flew across the room... $30 I'd never see again (in 1995 dollars!), woomp wooomp.

There's also the third party Dual Turbo wireless controllers that are styled in the classic three-button controller mold.
I have some other ones not on theclaw's list but I can't think of the names off the top of my head.

There's also variants you won't see in most places.
Blue 3 button pad with that Aiwa Mega CD stereo.
Samsung brand 3 button pads for Korea.
Brazil got gray and white editions of the 6 button pad.

This may be way off topic, but I saw a fellow at a local game store playing Sega Genesis with his smartphone.
The console had the infrared receiver for the wireless 6-button pad plugged into it.

Very nice. I wonder how he did that - I have tried Genesis on the smart phone and the emu is pretty good, but the controls suck. I would love to know what he used.
As far as controllers, can't help there I was thinking the Activator but then noticed it on the list.

I think it was just one of those universal remote programs that you can use to control televisions, AV receivers, etc.
