Specifically, he's designed and mass produced haramaki, i.e. belly warmers.
Don't stress if you don't know what these are. Haramaki are sort of like a mixture between a corset and a sweater, and they apparently used to be a laughably unfashionable item often associated with old folks. According to Itoi's company Hobonichi (http://hobonichilove.tumblr.com/post/94381807232/hobonichi-presents-official-nintendo-haramaki-belly), they were "once strictly functional underwear that had a grandpa-image similar to yellowing long johns." But then in 2001 Itoi started producing haramaki and seemingly willed them to catch on.
