That is The Legend of Xanadu by Falcom, and it's great. It has a really fun sequel as well, Legend of Xanadu II. It is part of their Dragon Slayer line of games that included Dragon Slayer, Xanadu, Romancia, Legacy of the Wizard, Faxanadu, Sorcerian, and The Legend of Heroes games. I enjoy them quite a bit!
