Apparently there was a class-action lawsuit over the month PSN was down in 2011. I got an email about it and you can read more about it many places, here's one: http://www.ign.com/articles/2015/01/23/psn-class-action-settlement-do-you-qualify
The game choices are pretty lame, at least for me as I have lots of them already. I selected Puppeteer. Anyway, just a heads up in case anyone didn't know.

Didn't Sony Give 2 games each and a free month of Playstation Plus?

I thought they did too, but seeing that I own (technically) neither of those 2 systems and I really see no value in giving away my information to get some lame themes I'd never use anyway I'm not bothering. If they put a $10 PSN credit towards buying something then sure why not.

I have Puppeteer free on PS+ already and own everything but Rain and some of the PSP games. Not going to bother.

I would have done it if I could do it digitally just for 3 months of ps+ free.
But I'm to lazy to print and mail this out, I also didn't have ps+ at the time so I'm not eligible. Ehh whatever

Do you have to have had PS+ during the outage to qualify for this? I don't mind filling this out if I can get a free PS3 download for it.

Read that thing carefully before you fill it out. There was a topic on Neogaf about it. It's amazing how many people clicked "sure, give me money" without noticing the "we'll also be closing your PSN account" lines.
