This is a bit unorthodox, but what games feature the Grim Reaper? These are what come to mind:
Castlevania (the only games excluding him are the first two game boy adventures and Haunted Castle)
Shadowgate (It's a sad thing that your adventures have ended here!!)
Kid Icarus (you face him too soon, IMO. If you can get past him in the first stage, it's clear sailing from there on.)
Maximo (I'd like to play Army of Zin to see how he controls in this game)
Monster Party (Welcome! Entrance to Hell!)
Gauntlet (to quote Abe Simpson, AHH! DEATH!) LOL
Any others? I'm sure this isn't all there is. That skeleton guy is everywhere, even... RIGHT BEHIND YOU!!! Yes, that was a cheap attempt for scaring you, and I'm sure it failed. :P

Champions of norrath has a enemy that looks like death but he is not. Is death in the Ghosts and Goblins series?

I'm sure that the Reaper has appeared in a lot of other games, but I can't think of them off the top of my head. A lot of older PC games come to mind, like Spellcraft: Aspects of Valor (where you visit the Death realm), or some of the Might & Magic, Bard's Tale, Wizardry, Ultima, and other games. I'm sure some of them have the reaper on the death screen. I can't find out without playing all those, though - doesn't look like this information exists on the web...

Numerous Death-like enemies called Skeleton Murderers do show up in the first and fourth stages of Ghouls 'N Ghosts. Their role in Ghouls was the same as the zombies from Ghosts 'N Goblins and Super Ghouls 'N Ghosts. They also pop up again in Makaimura For Wonderswan, along with the zombies - the only G'NG game to my knowledge to feature both.

That's one right there. There is no Death boss in any G'NG game, though.

Depending on the height in which the scythes are thrown, they can be jumped over or ducked under. Get close and whip away, as Zeldos take plenty of hits."
On a side note, you know the Faces Of Death (http://www.classicgaming.com/castlevania/dracform/grimform.htm) section of the Dungeon, the one that covers all of Death's various forms in the CV games? I was the one that gave Kurt the idea for that section, as well as contributing the startegy for beating Death in CV1.

I wasn't counting Zeldo, actually, but I knew their resemblance to the GR was obvious. Not as tough as Grim himself, but they are among the toughest enemies in the game. And ever since using that strategy, I never had a problem with Grim ever again... unless I was lacking Holy Water. :D

And I almost forgot one here... maybe it's not GR himself, but the third boss of Gaiares, which looks totally out of place in the game, is a giant Death that keeps firing sickles at you. Not sure what Telenet was smoking when making that boss... hell, that entire stage was wacky. How many castles are out in the middle of a dangerous, black-hole-infested sector of space anyway?

Yup, that is where they came from.
I wouldn't count Zeldo as a Death appearance either, even though he looks similar. I do remember that boss you mentioned from Gaiares though. I'm not 100% sure he is Death himself, but he looks just like him.
To Flack: Grabbed By the Ghoulies is an Xbox game.

It's not just a giant Death, it's a giant robotic Death. Coolest boss in the game, imo.

Besides Rare's Conkers... Rare also has the Reaper in Grabbed By The Ghoulies. Its cool because in rooms where you dont kill the bad guys fast enough and Grim appears(when he is the effect in that room) you can use his powers to get out of the room. Just place them baddies in front of Grim, one touch and they are dead.. getting you out of the room much faster.

Golden Sun, while it doesn't have an appearance of the Grim Reaper, mentions him when a particular spell is cast.

I just bought the game and have been playing it, and so far, I haven't seen a Grim Reaper. (Which is kinda a surprise)
My guess is Ghoul Patrol, which I haven't played.

Dragon Slayer (GB-Japan) has them. Actually, I believe the Instructions Call them sa-ta-n (katakana). So, while the look like Grim Reapers, they are Satan...hmmmm.
