What does the cart add-on slot do? Also, if I buy it, can you give me instructions on how to install it?

It slots on where the megacd usually goes (this is where it takes the power I assume). You put the cart attached to it into the magdrive/genesis cart slot then you fill up the 6 slots with games. Each time you press the switch, it changes to 1 of the other carts, kind of like a cd multichanger, but for megadrive/genesis hehe. I'd tell you how to work it even if you didn't buy it :) (but this post pretty much explains how anyway..)

So it's compatible with both Megadrive AND Genesis games?

It's compatible with anything that you put in the cart slot, whether it be master system converter, 32x etc or normal megadrive/genesis games - it has no region lock, it's just a bunch of cart slots!
