I was playing through Streets of Rage 2 on the Master System and noticed quite a few changes. Gameplay was the same, but a lot of background detail was removed. I always though the systems were identical, so why the change? Also, what games do vary wildly from each other if they were released on both?

The Power Strike/Aleste games are different games entirely.
Wonder Boy 3 wasn't released in Japan originally so when it got a GG re-release they gave it a fancy new title screen and ending credits.
Some games that appeared on both got slightly downgraded on SMS(Virtua Fighter Animation, for example) because the hardware isn't as advanced(less colors) but most are almost the same.

The major difference between the SMS and the Game Gear is that the Game Gear has a larger color palette to choose from (and is capable of displaying more colors onscreen).
As for Wonderboy 3 / Monster World 2 the GG port's levels are slightly rearranged to accomodate the smaller resolution. I believe some of the swords and armor are acquired at different points too.

Sonic 2 has a difference, when you fight each boss, it has totally different BGM compared to the GG, plus the title screen has no BGM and the intro is cut short.
Yet the plus side, is the screen resolution is better for Sonic 1 2 and Chaos, so you can see secrets and other platforms that you'd normally miss
