I purchased this a couple months ago and never used it because I no longer play my PS2 games online so I gotta give it up to someone who can really use it. It includes all of it's original contents. The FFXI Disc has only been inserted into my PS2 once so the disc is flawless. Here are the contents:
FFXI Online Game w/ PLay Online Disc
PS2 40 GB Hard Drive(HDD)
Installation Discs & Manuals
Anything else I forgot to list
I will format the HDD before it's shipped out.

Since the box is pretty hefty, I would have to say shipping would cost me around 6-7 bucks(hopefully). As for the price of the bundle, I'm looking for 52$ shipped.
I also have a cosmetically flawless Official XBOX Big Aka Duke Controller for sale. It includes the Break-A-Way cable as well. Shipping's gonna cost about 5$. I'm looking for about 30$ shipped for it.
I accept Money Orders of any kind but I do not accept personal checks. A Money Order usually costs around .80 cents at the Post Office so everyone is eligible to get one.
