I didn't have to kill for it though. Just filled out an application.
NextCard (now defunct) had a card which you could customize
with any image, by uploading to them. Although you only got about
⅔ size of the card. Still 8-)

They had an ad for these Nights into dreams and other SEGA related phone cards in the end of year 96 wrap up EGM2... yeah.

You'd think after all his posts he would know how to post an image.
If you insert the image directly, you're stealing bandwidth, so he behaved correctly :P The post could've been more descriptive though of course ;)

Phone cards are a dime a dozen for any major game release in japan. Just check out these Grandia (http://www.geocities.com/grandia_shrine/index.htm) ones (go to the product section).
