I'm looking for suggestions...I've played most of them, looking for something I haven't tried yet. I'll start it off:
-Fatal Frame II
-Silent Hill 3
-Silent Hill 4
I have the following systems: PS1 (domestic or import---I'd love some import suggestions!), PS2, GC, DC, and a bunch of older systems.

Play the original Clock Tower for PS1. Sure, it's old but that game still scares the shit out of me. I have never experienced the same kind of panic I felt from being chased by Scissorman in another video game before.
Clock Tower 3 was good but I thought 2 was terrible. Was too different than the other CT games. Definitely play CT 1 though if you can find it.

Cannon Fodder always gave me nightmares. Not because of the images, mind you, but of the enemy soldiers screaming if you only wound them.
A video game featuring Salad Fingers would be MOST disturbing. X_x

Haven't you played the original Silent Hill PS1? That still bugs me out.
To me, the most bizarre thing I've seen, is in Resident Evil (GameCube)....Lisa Trevor...this poor tourmented girl in chains, wearing dead skin on her deformed face like Leatherface would.
Clock Tower (SFC). When you find the bathtub, you hear water tripping...and you look into the tub..and scissor man pops out. Scared the shit out of me...
D (Saturn/PSX) First time you see Laura look into the mirror...she touches it...and the hand pops out trying to reach out to her....
Or when Laura opens the closet door to her left as she walks up the stairs, there is nothing but spikes and a dead corpse. Then when you see the painting of the little boy, it just grins and laughs.
Plus seeing the dead body in the dungeon room, yet it's right hand is cemented into the wall. So when you look at the opposite side...there's a weird box...once you unlock the puzzle to it...there's the corpse's hand with the ring on it...
Yet to break it down, most disturbing game is Silent Hill 4: The Room.
When a stuffed toy bunny is pointing at you, making a gesture to "Pick up the phone" that scared the piss out of me....Or in the beginning when Henry has bizzare visions of these creatures coming out of the walls and celing....mind boggling...
Kicker was seeing an ambulance carrying a body away from the subway enterance...then the next day... seeing the girl, that you talked to, she's attacked by dogs, but there's no way to help....then the next day, you see her walk down stairs to the subway enterance while you looked out the window from your apartment...so you follow her.....yet when you find her....she's already dead...
