I saw this on ebay
Notice the Splatterhouse 2/Super Monaco GP Cart!
Im 99% sure its a pirate, however it looks authentic with the Megadrive logo at the top...

Yeah, almost certainly - either that or a home-made inlay. Note how it's in a VHS-sized case rather than a standard MD box (*) and that the logo isn;t quite centered properly.
Odd that he doesn't mention Splatterhouse being among the games either.
(*) Having said that, the Ozisoft 2-in-1's are also in VHS sized cases, but they look very different to that.

While it's pretty much already been said, that definitely looks like a pirate of some sort. The cover alone reeks of something I would've cranked out in photoshop on a bad day.

Strangest combination of games, even if there was a chance it was original, I couldnt see it being a combination any real games publisher would put together.
Theres also the possibility that, its one case and two original cartridges inside.
