I could've sworn that at some point I saw a copy of Pac-Man World 2/Pac-Man Vs. that WASN'T Players' Choice. Am I remembering incorrectly, or does this actually exist?

Nope, it doesn't exist. All PC versions of PMW included PMvs and PMvs wasn't out until PMW went PC. Stores were sent PMvs in a cardboard sleeve to give out with any non-PC PMW copies they had.
