Azure Dreams Konami Addendum?

Hey all I just got back for a local game shop. While looking at a copy of Azure Dreams for PSX A slip of glossy paper fell out titled "addendum". It has contact info for Konami, info on their hint line, and a message that 3D glasses and The Justifier gun are no longer available from Konami. I bought azure dreams new and I'm pretty sure my copy didn't have one. Is the "addendum" common or did they just slip it in the last batch of releases? Anyone know?

I don't have it on hand, but I don't recall mine coming with it. I bought my copy used, though, but it was really mint.

I can provide a pic if anyone would like. Now I'm even more curious as to how rare it is.
