Albuquerque, NM:
The Last Bounty Hunter
Mad Dog McCree
Mad Dog McCree II
The footage for these games was all shot in or around Albuquerque, which was home to American Laser Games. I don't know if any of their non-Western titles e.g. - Who Shot Johnny Rock? - were also filmed here but it's possible.

I was living in Akron, Ohio when No One Lives Forever 2 came out and was caught by total surprise when Cate goes on a mission TO Akron, Ohio. Talk about totally random.
Downside was the most of the action took place in a trailer park :( Not quite the Akron I remember being in...but hey, what the hell. It's better than nothing.

DC:
Midtown Madness 3
Various games in which the White House get blown up in a cut-scene.
Seems like there should be more.
Edit: 'Course I live in Pittsboro, NC now, so not much going on at the moment. Closest thing is Raleigh which is named after Sir Walter Raleigh who, among other things, was a privateer in the veign of Sid Meir's Pirates!. It's a bit of a stretch, I know.

Hometown (Plattsburgh, NY): Nothing
Current Home (Mobile, AL): Get to destroy it in the original Rampage game (Arcade, NES, Multiple Midway compilations) even though the cities look NOTHING like their real-life counterparts. (For the most part anyways.)

Well, Montreal doesn't appear in any videogames (save for sports games, of course), but there is an in-joke pertaining to a local BBQ chicken chain in the first Splinter Cell game, which was developed by Ubisoft Montreal.

The closest for me would be either Joilet or Homewood, IL from Rampage. (Neither of which has skyscrapers, but I digress...)
I'm sure Chicago has been in a bunch but I can't think of any off-hand. :/

Seems like there should be more.
The first campaign of Jungle Strike takes place in Washington.
Rampage is the only one I know of with mine (St. Louis). Montreal is in it too, Lady Jaye.
Chicago is a level you can purchase later in the game in Tony Hawk 4.

For Portland, OR:
-The first Tony Hawk's Pro Skater includes the infamous Burnside skate park.
-Rampage World Tour has a Portland level.
-Big Rigs Over The Road Racing (that crappy PC game that won Gamespot's worst game of the year award last year) claims that you drive from Portland to Miami, but with how misleading the game is I doubt it.

I'm in Roseville, MN...nothing there.
However, Roseville is next to Minneapolis, which is in Rampage and Tony Hawk's Pro Skater. The Ice Cap Zone in Sonic 3 might be based on these parts during the winter as well... :hmm:

Games set in the UK are a big enough deal! :P
As for Milton keynes, despite it being a pretty big town, all the gaming actions happens in Londonzorz

:?
Got it! "Campaign 1984" for Colecovision. Anybody else needing a fallback answer can use this one too as it involves all 50 states (yes I know we're dealing with cities but let's have a little slack here).

Not sure of any others, but Tulsa, OK is in Rampage on the 7800. I remember playing late one night and seeing it and getting all excited. Hey, I was like 10 or something...cut me some slack would ya!
I can't think of any other games to have my home town in it besides this.

Livermore, CA:
Metal Gear Solid (2?)
Only mentioned in passing, Lawrence Livermore National Labs. My father works there, and my credit union is on the premesis...
They also filmed part of Tron there, in the building where they used to have with the world's largest laser. (the scene where the guy gets zapped into the computer) Of course, they tore it down to build a building with an even larger laser in it...
