In Dynamite Headdy one of his expressions is to curl the arrow sprouting from his head into a question mark, with his head serving as the 'dot.' This comes up fairly early after the second boss fight in a cutscene.

They have a ? for the confusion power in Psychonauts.
In Megaman, I can't rememeber exactly which one, I think 3 for sure after you beat the first 8 bosses. They have capsules with ? on them, and when you shoot them, their is a regular power-up inside.

In Monster Party on NES ,after you kill a boss, a ? appears which you can grab for a random power-up.

There's the '?'s that appear over the heads of the guards in Metal Gear Solid when they can't find the source of a noise and such. Poor, confusable guards.

There's some game where little question marks appear above character's heads at points in the dialogue, and exclamation points too, which sometimes fly straight up in the air before landing back on their heads. I can't remember what it is, though. Final Fantasy 5?

In Final Fantasy VII, the Enemy Skill known as Pandora's Box was pretty much a gigantic question mark.

In the Tenchu series, ? [or something like that] is displayed in the Ki meter when the enemy hasn't sensed your presence.
Crash Bandicoot on the PSX, all of them... The boxes have question marks on them...

NES Exodus: In the action part, you need to collect five question marks and enough Manna to open the exit of the maze-like screens.
