there are a few action/platformers with a wolfman character. i'm surprised there hasn't been a survival horror title, something like the movie Dog Soldiers.

Yes, I specifically remember the NES game Werewolf that came with a comic...
Has there been no werewolf in any Castlevania games? I don't recall any...odd.

CasteVania 1 doesn't
CastleVania 3 Dracula's Curse doesn't have any werewolves
Super CastleVania 4 doesn't
CastleVania Bloodlines doesn't
The CastleVania Adventure on GameBoy doesn't

Project Altered Beast for PS2. Know that altered beast is already mentioned, but I guess that was the Mega Drive version :)

I didn't know this game was also released in cart format (also not about the SNES version). As in Europe we only got it on Game Gear, Master System and Mega-CD.
Anyway it was the first game I thought of with werewolfs.

Silverload (PlayStation) has werewolves, but you play as a bounty hunter.

Wolfchild also had an Amiga release. I too thought of that game first ^^

Oh, right! Of course...can't believe I forgot that...
No that wasn't sarcasm, but it actually would have been kind of funny if it was, LOL

All of the Bloody Roar games had a werewolf character. Also AD&D Iron and Blood had a werewolf character. Pretty sure J. Tailbain was in all of the Darkstalker games and he was a werewolf.
Fighting games love the lycanthropes.

The Legend of Zelda: Twilight Princess (in its current state) has Link become a werewolf.
DarkStone(PS): the wizard(?) character has a Lycanthropy skill.
Blood Omen: Legacy of Kain(PS): wolf form "allows Kain to jump over otherwise impassable obstacles & ledges, move faster, & rip open throats."
Gotta love it.

Is Ralphie from King of the Monsters a Werewolf? Or just a plain wolf?

I'm fairly sure 1 of the Nightmare Creatures titles had something important to do with werewolves.

The first one definitely (not sure about 2). They like to break out of enclosed areas and rush you. I don't like them.
