Looks real to me. I own some White Lables Samples from the Dreamcast too which look the same. But I would not count them as Prototypes.
I am always not sure, if these are complete games, because on the Lable stand always SAMPLE. For me it sounds like a DEMO. But I have not try long enough my Samples discs.

These White Lables Sample discs are easy to find I think and there exist many from them. Not sure about this special game. And its very interesting, when its not released. But I think its no Prototype. Just an unreleased White Lable Sample Disc. So more like an unreleased Promo for PlayStation or PlayStation 2 when there exists something like this for these consols.

I own these White Lable Samples for Dreamcast. And I have get all very cheap I think. Some have I get from the IFA (an Electronic Convention in Berlin) for free, Sega just give them away.

The fact that the only reason this game is anywhat valuable is that it was unreleased does not make up for the fact that it sucks.

It looks legit and all, but look at his feedback and read some first, cause hes got %96.3

I've played it, it's appaulling - not much of a game.

There was a guy flogging them for £70 a time then he got a dumb amount for one so the rest shot up in price.

The white labels came out in europe, not easy to get not hard if you know what I mean, if you knew a shop worker, journalist or press guy.

Prices seem on the rise slowly, not sure if they are worth bustinga gut for.,

I'm certain Big Ben, who were distributing the DC at the time had white labels of propeller arena at ECTS 2001.
