I thought it might be a Deck Enchancer (http://nesworld.parodius.com/aladdin.htm), but it's too small for that.

I guess it might be a Game Genie (http://nesworld.parodius.com/gamegeni.htm) from outside of North America (as Codemasters did not distribute it there).
