How the hell does this thing go in here?

I just picked up a used copy that has apparently been in my local gamestop since November of 04, and it's got a bunch of documents, the 4 pieces for the controller, and the game. It has one cardboard divider that is very shallowly intented running the length of the piece. Right now all the pieces are just kinda thrown in there and I haven't looked at it much, didn't have any time before work, but I'll be damned if I have any idea how this goes in here.

I'm likely missing pieces too, so any info on that would be super.

Wow, I guess I could have searched first... didn't think it would have already come up though, heh.

Well, hrm... I just checked out those pics and the only piece I have is the top piece that the game and documents are resting on. I don't have any other pieces that the actual sections of the controller rest in. Don't suppose anyone would have any to offer? :(
