That's the Victormaxx Stuntmaster, one of the earlier attemps at an inexpensive HMD. You can read some info on the devolpers page here (http://www.frerichs.net/fvt/stuntmaster.html)

I bought one of those off of eBay like 5 years ago for like $15 shipped just to try it out. It sucks. Its like an extremely low resolution LCD that's totally washed out and using the device as a controller is a joke. After I realized that, I modded it to take any composite A/V source (mainly to try with my DVD player) and it still wasn't worth it. So off to the trash it went.
