Help with a couple of nintendo power questions

1) what months did issue 13, 14, 15 come out?
2) what game was on the cover of issues: 28,69,86,143 and the different covers of 177?
3)did issue 68 have 2 different covers " batman & Robin and '95?"

A few months ago I said I was going to get a complete list on Excel of the first 200 issues, including variant covers.
The list would have the date,issue number, and the name of the game on the cover. I dont have complete collection by a longshot, but figured this could help me and others start collecting. Any help would be apprecaited. I will post more questions as I have them if that doesn't bother anybody.

If you were a subscriber, then yes, it came with the nice glow-in-the-dark '95 cover. I highly doubt it was packaged in that fashion on newstands (if it was ever on newstands).
