Anyone heard of that? Presumably only for Japanese stockholders of Konami. How rare is this game, on the DP scale?

Yes, there was also one for MGS and MGS2, I believe. As far as rarity goes, being located in the US, it is hard for me to tell since they are Japanese. I would say that they are attainable, but fairly uncommon. I would say about a 5-6 on the DP scale, but that is a rough guess. They certainly aren't more common than that.

Mainly cosmetical. The PSX Premium Package Shareholder version is gold instead of silver, and MGS2/3 have a version with a different DVD case. I don't think there is anything extra. I'm not exactly sure how they are distributed either.
