I received an email from Nintendo titled "Important Safety Information for Playing Wii". It has new pictures but the same precautions in the manual:

wear the wrist strap,
don't let go of the controller, and
make sure you have enough space to move around. It probably is the result of this:
http://news.yahoo.com/s/ap/20061207/ap_on_hi_te/japan_nintendo_wii

and the Wall Street Journal article about "Wii elbow".

Having purchased a rubber grip for my Wii (heh) I can say these things can still fly out of your hand easily. There really should be some kind of rubber strips on the side because the plastic used is beyond slippery. I've lost mine once playing Wii Sports baseball and almost nailed my friend square in the balls with it. Been paranoid ever since.

My wrist strap has stayed in place without any issues however.

My wife came close to throwing one, the strap did stop it. I haven't had any problems but it sounds like people are really getting into the games, Wii straps breaking is starting to happen quite a bit.
