I've only seen the following at their locations for the past several years, both in Pelican (the manufacturer) and GameStop branded versions. I've never seen the model in your attached photo.

The last time I saw that 5-port one new in the stores was about six years ago. I had a Pelican-branded one back then.
They're pretty common on that auction site if you can't find one elsewhere.

Yeah, I'm sure there's lots of them on the Net, just mine is on the fritz and wanted to get one tomorrow. The one I mentioned is actually a Pelican unit that GS rebranded.

I found this cool neat little thingy at fry's,
    http://www.frys.com/product/5886963?...H:MAIN_RSLT_PG

I'm really not sure if it only works with sony systems, but I'm gonna try it out soon, as i picked one up awhile ago. It has 4 AV plug in spots, one is the flat style, and the other 3 are the either S-video, or the standard R/W/Y plus style, basically giving your a choice on 3 of them. I don't have a dreamcast, or xbox systems, but I can try everything else I got and see if ti works.

Yeah that was designed for the fat PS2, because it doubled as AV switch and PS2 controller multi-tap. Doesn't help me, because I need 5 inputs, just have to browse ebay.

Do you still need one of these?

I think I may have one somewhere around here, you can have it for the cost of postage if I find it.

Well on the box I own, it says for the new designed ps2, and shows a picture of the slim. So.....

Hi, thanks for the offer, but I found one still new on ebay for $10 yesterday.
