According the the guide the game is actually about diabetes. My guess is that this version was released for use at doctor's offices instead of a normal retail release.
oh word, my girl has diabetes, maybe I should get it.
From the auction description:
So it appears that the major difference between the DO release and retail would be the addition of the cardboard sleeve.this game comes just like every other SNES game except it also has a cardboard sleeve that was intended for doctors office use.there are areas on the back to write loan out dates and names.
Huh. A very interesting cartridge. Was it ever released in stores? I wonder how many were made. Oh, and by the way, a lot of doctor's offices have video games to help kids to get over initial anxiety of going to the doctor's office.

Pretty cool find. I'd never even heard of this before.
Yeah, it is a sleeve that goes over the box. Definitely quite rare compared to the game.
Edit: wow, $51. That is quite a bit.

The smirt it guy must be a deep pockets collector..he's got several high bids on some of my stuff.
