It apparently says NTSC on the front, but is in PAL style packaging? Is this just a cdr where someone has printed off a copy of the cover and pasted "NTSC" on it?

Or did the NTSC version actually come like this?

I'm pretty sure you're accurate in your guess about the homemade packaging. The U.S. release of DE came in a garden variety longcase. I don't know if that's what the mega cd cover looked like.

Couldn't it be the Japanese version? Since it's called 'Mega CD' over there as well?

That would be my guess as well. It's hard to tell from a picture, but that looks like original shrinkwrap to me.

In Japan, they use NTSC video and called the machine the Mega CD.

No. As was pointed out, it's in PAL-style packaging.

Asian? I've seen Asian Megadrive releases in PAL packaging.

Most of Asia uses PAL (except for Russia (SECAM), Myanmar (NTSC), and Japan(NTSC)). I don't think they're playing many games in Myanmar.

South Korea is another NTSC country. I don't think they're playing many games in North Korea (Missile Command?).

Whoops. Guess I missed it on the map here:
    http://en.wikipedia.org/wiki/PAL (North Korea's PAL, BTW)

Good eye, Vroomfunkel...that can't be right. Maybe it's just an NTSC sticker? You can rule out it being a sign for the seller...

It's also different to the standard Euro version in that it seems to be in a single CD case rather than a double.
I'm inclined to agree with it possibly being an Asian release - some Asian MD games were also released in both PAL & NTSC formats.
