I have more R5's than I realized, but I managed to find an R7 in my collection - Quattro Sports (NES).
There are two versions of Quattro Sports for the NES. One goes into the Aladdin Deck Enhancer and the other is in a shape of a regular NES cart (well, the shape of a 'regular' unlicensed NES cart). The more common of the two is the latter, which is an R2, and its gold.
Aw crap... then I think I have the R2 version.
That would make my rarest game an R6 - Ultimate League Soccer for the NES. Found it at a pawn shop.
Hmm....good question. It could be Snatcher for my Sega CD, Tengen Tetris for my NES, the Japan-only Star Wars Famicom game, Silent Hill for the Japanese GBA, Perfect Dark for the import N64, or Castlevania: S.O.T.N. for import Saturn. Now that I think about it, my God of War 2 E3 demo(taken straight out of the E3 PS2's used) would definitely be the rarest. Too bad it's not a full game.
...Honestly, I dont know. It would be one of the following:
NES Metal Gear, mint, with mint box and all inserts, map, manual, etc...
Panzer Dragoon Saga, complete as above, mint
or Princess Crown, Saturn, complete, mint.

I have some great items but when it comes to rarity I would have to say it has to be the artwork. Even if an item appears to be one of a kind sometimes another pops up. These are the original paintings used for the box art so there can not be another. I have close to 500 pieces but 100 or so are paintings which are by far my favorite.

My rarest item is probably the MACS Rifle Simulator for the SNES. Too bad it's missing the rifle itself...

My (I think) only R8 is Starship for the Vectrex (aka Euro Star Trek). I also have a complete JAp Shining Force 3 scenario 1-3 and a loose premium disc (which I'm still rather proud of, incidentally). Nothing really Earth-shattering though.
I don't own many rare items, but definately my rarest is my complete in box Game & Watch Mickey Mouse DC-95.
Hmm....I don't have any prototypes or anything, but I'd say my rarest things include my boxed ZX80, my sealed PAL F-Zero on the SNES, Home Alone on the SMS, Mutant League Hockey, Zero The Kamikaze Squirrel, Flink and pirates of Gods and Dragon Ball Z on the Mega Drive (I didn't pay for them, got given them at my old work) and a pirate of Pokemon Crystal for the GBC (it's a weird crappy platformer, again got given at my old work).
Oh, and DS Demo carts of Pac Pix and Nintendogs. And one of those puzzles that goes "bzz" when you guide a metal loop around a frame, but in the shape of a hand for the DS release. Oh, and a Sega Channel t-shirt.

My rarest item would be my complete copy of Psychic Detective for the PlayStation. I am in the process of selling my copy of Sammy Lightfoot for the Commodore 64.
