According to me and my extensive knowledgeX-Com is not even close to one of my rarest games.
I'm going to say, and this is purely from a supply standpoint that my rarest game is...
P.T.O. for the Genesis which is listed as a lowly R5.
I've got three different diagnostic carts for SNES. When I asked a friend what good they were since you couldn't actually play them, he said you can play Super Nintendo Repairmen, so I guess they do count as games.
A few of my more difficult to find include things like....
Flintstones 2, Bubble Bobble 2, Tengen Tetris, Action 52, Stack-Up Complete, Chase the Chuck Wagon w/Manual, Star Fox Weekend Challenge Comp Cart, Fun n Games, etc..

I got 3 R7's for NES: Big Nose Freaks Out, Blackjack (boxed), and Maxi 15 (version with Double Strike and Pyramid).
Since N64 games only go up to R5, I got these: NFL Blitz 2001, NFL Blitz Special Edition (sealed), Razor Freestyle Scooter (sealed), and Worms Armageddon.
And this one's not in the rarity guide: Star Fox demo cart for SNES.
My rarest games according to your list are:

But I had a much harder time finding games such as Chocobo Racing (PSX), Kurushi (PSX), ...
Probably one of my Working Designs titles, most likely Lunar II for Sega CD but I have several imports that don't have complete entries in the rarity listing too so I don't know.
