I've got a Japanese Duo-R system, and I'm thinking of getting a controller extension cable. I noticed that turbo zone direct has a Duo extension cord for sale. My question is can I use that US Duo cable with my Japanese Duo system and controller?

yes. i have a JP Duo-R as well and i just recived a duo extension cable from TZD a few days ago. it works just fine

They're some old mac cables, but the connector is identical! I bought two for my US Duo a while ago. In fact, judging from the pics they look like the exact same product, only cheaper .
actually, now that you mention it, I believe, that is a eight pin out so that mac cable just might work, and the price it right to try... sweeet find.

All the TurboDuo (USA) cabled accessories and ALL the Japanese PCEngine cabled accessories (doesn't matter which system) use the same jack. The TurboGrafx-16 was the only odd machine out. I have a Japanese "STICK Engine" made by ASCII and it works perfectly with my TurboDuo, imagine a black and green NES Advantage.
PROTIP: Pc-Engine / Turbo DUO controller cables are identical to S-Video cables.
As are the old AppleTalk/Printer/Modem Cables.
You can use S-Video extension cords.
It's also highly likely you can gut a S-Video cable and use it to convert TG16/PCE pads to the DUO (I'm going to try this over winter break).
ALSO, TZD has been around for over a decade, they're good folks.
